PR3

Autoantibodies to PR3 (short for Proteinase 3) belong to the group of anti-neutrophil cytoplasmic antibodies (ANCA) and are also known as PR3-ANCA or cANCA. They play an important role in the diagnosis of granulomatosis with polyangiitis (GPA) (formerly known as Wegener's granulomatosis). PR3 is a serine protease with proteolytic activity that targets a variety of substrates and plays a role in tissue remodeling during inflammation. It is found in neutrophils and in the lysosomes of monocytes.

GPA is a type of vasculitis, a family of rare diseases characterized by inflammation of the blood vessels that can restrict blood flow and damage vital organs and tissues. GPA typically affects the sinuses, lungs and kidneys, but can affect any organ. The diagnosis of GPA can be challenging due to its diverse symptoms. Therefore, a combination of several test including imaging studies, tissue biopsy, urine tests and blood tests searching for PR3-ANCA is performed. GPA can get worse quickly, so early diagnosis and treatment are essential to prevent organ damage or failure. PR3-ANCA are found in 80-90% of patients with GPA.
